Plan 1: Basic Membership

- 20% off all office UCR fees
- Additional Family Member: $50
- Up to 4 members per family can be added (each additional family member will pay $50)
Plan 2: Comprehensive Membership

Benefits:
- 2 Exams per year
- 2 Sets of X-rays per year
- 2 Adult Prophylaxis Cleanings per year (in absence of periodontal disease)
- 20% off all office UCR fees
- This plan is only for individual patients.

Adult Prophy: $95.00 – 20% discount = $76.00
